Working with Other Agencies

Our key workers work in partnership with Social Workers by being involved in meetings, contributing to formulating young people’s care plans, and being involved in the pathway planning process of young people placed at Pathfinders.  Our key workers share information with Social Workers on regular basis and also provide weekly and monthly reports are and when required. 

Our key workers have a responsibility of notifying EDT, Police, and Social Workers when a young person goes missing from the placement.  Pathfinders’ key workers and managers attend strategy meetings when young people are reported missing from care or when a young person is deemed to be at risk of harm or abuse ie risk of CSE, gang affiliations, etc.
